Job Summary: The Clinical Pharmacy Coordinator analyzes physician prescribing, recommends alternative cost effective therapies, develops intervention processes to improve quality and cost minimization of prescribing, creates educational programs for physicians, and provides drug information to clinicians within the Sutter Health system. The position is responsible for analyzing drug trends and creating interventions to assist in decreasing overall healthcare costs, developing and funding educational programs, providing pharmaceutical information within the disease management programs, and developing clinics to assist physicians to improve the quality of care of patients. This may include working with providers on medication therapy management and direct patient care. Accountability 1 - Program Development * Create customized quality and intervention programs and associated materials * Prioritize intervention programs related to medication use * Develop intervention program template, including analyses, materials, and clinical support information * Create customized intervention process to improve quality of prescribing and decrease healthcare cost Accountability 2 - PAMF-wide Formulary Management * Work with pharmacy and medical leadership to develop schedule and appropriate resources for medication and therapeutic class review * Present clinical and financial information to Pharmacy and Therapeutic (P&T) Committee * Coordinate the summarization and distribution of P&T Committee decisions * Work with pharmacy and medical leadership to implement P&T decisions * Measures the results of the decisions and recommend modifications to increase the effectiveness of the program Accountability 3 - Safe and Effective Medication Utilization Management and Evaluation * Complete clinical review and analysis of medication and therapeutic classes * Complete research on medication use with the system * Coordinate appropriate meetings with clinical experts for formulary reviews * Make appropriate recommendations on medication and therapeutic classes for P&T Committee * Distribute education materials as part of therapeutic intervention programs and new drug therapy guideline Accountability 4 -Collaboration on Improvement Work * Assist with Quality and Patient Safety initiatives on quality improvement, risk and safety activities by providing consultation from pharmacist perspective * Assist with Service Line pharmacy related quality improvement efforts * Maintain Service Line engagement on cost-effective pharmaceutical utilization efforts Accountability 5 - Clinical Guideline Involvement * Assist with clinical guideline, policy and protocol development in collaboration with various committees, physicians, nurses, staff, and pharmacy related administrative leadership Accountability 6 - Commitment to Excellence * Follow the commitment to excellence and treats patients, families, visitors and each other with courtesy, dignity, respect, and professionalism Required: Bachelors. California Pharmacy Licensure. Desired: Masters, Pharm D, Board of Pharmacy Specialty (BPS) certification and/or pharmacy residency Experience: Prior in-patient, out-patient, or clinical pharmacy experience is required. Prior experience developing clinical programs, presenting clinical information, communicating drug information, and calculating the cost/benefit of medication use and formulary decisions is desirable. Knowledge: Extensive clinical knowledge of medical and pharmaceutical issues is required. In addition, expertise in clinical interventions is recommended. Knowledge of managed care, hospital and physician group issues is strongly desirable. Financial analysis knowledge is highly recommended. Skills: Proficiency with Microsoft Word, Excel and Power Point is required. Familiarity with pharmacy claims data systems is desirable. Excellent organizational and time management skills, and strong communication skills, both written and verbal as well as interpersonal skills are required. Professional presence, capability to work independently and ability to establish work-flow priorities with minimum supervision are important.
