Description Clinical Pharmacist at Providence Santa Rosa
Memorial Hospital in Santa Rosa, CA. This position is per diem and
will work 10-hour, Day shifts. The clinical pharmacist I has the
responsibility for the provision of high-quality, individualized,
cost-effective pharmaceutical care and integrated pharmacy
operational services in collaboration with the healthcare team to
optimize patient's medication therapy outcomes and quality of life.

we must empower them. Required Qualifications: - PharmD - Pharmacy
Doctorate or - Bachelor's Degree in Pharmacy - California
Pharmacist Registered upon hire - Basic Life Support (BLS) upon
hire - where applicable Preferred Qualifications: - Acute care
hospital pharmacy experience, 1 year or equivalent experience. -

position. The successful candidate's placement on the wage range
for this position will be determined based upon relevant job
experience and other applicable factors. These amounts are the base
pay range; additional compensation may be available for this role,
such as shift differentials, standby/on-call, overtime, premiums,
