Job Description Job Description Job Overview The Lead Care
Manager is a key healthcare professional within the CalAIM Enhanced
Care Management (ECM) program, dedicated to supporting high-need
Medi-Cal members. This role focuses on coordinating care,
addressing complex medical and social needs, and ensuring patients
receive the right services at the right time. Unlike traditional
care management, this position is community-based and highly
mobile, requiring frequent visits to patients in their homes,
shelters, hospitals, or even on the street to provide personalized,
whole-person care. Job Duties (NOT limited to the following) Serve
as the primary point of contact for patients, coordinating their
medical, behavioral, and social care needs. Conduct home visits and
field outreach to engage patients where they live and access
services. Develop and implement individualized care plans,
collaborating with patients to set and achieve health goals. Work
closely with healthcare providers, social workers, and
community-based organizations to ensure seamless care coordination.
Assist patients in navigating Medi-Cal benefits, scheduling
appointments, and securing necessary medical services. Link
patients with critical community resources, such as housing
assistance, food programs, and transportation services to address
social determinants of health. Provide telehealth support when
necessary to maintain continuity of care. Advocate for patients by
identifying and removing barriers to accessing essential healthcare
and support services. Must Have Skills/Qualifications Must have an
MA Diploma or CNA certification. Strong ability to problem-solve,
build relationships, and advocate for patients in complex
healthcare systems. Comfortable in a field-based role, traveling to
meet patients in various settings outside of a traditional clinical
environment. Passionate about whole-person, patient-centered care
and improving health outcomes for high-need individuals. For
