This role involves working across our inpatient rehabilitation hospitals and home health agencies to provide expert occupational therapy services, designed to foster patient independence and recovery from injury, illness, or surgery. Conduct patient assessments to determine their functionality and capabilities. Develop personalized intervention plans to promote patients' health and rehabilitative needs. Implement treatment programs using therapeutic exercises and activities. Evaluate patient progress and adjust treatments accordingly. Document patient care and outcomes to ensure comprehensive data is available for therapy continuity. Consult with other healthcare professionals to optimize patient therapy plans. Educate patients and families about home health care strategies and injury prevention. Strong skills in patient assessment and rehabilitation. Proficiency in developing tailored care plans. Ability to handle therapeutic equipment and apply proper treatment techniques. Excellent communication and interpersonal skills Capability to work collaboratively in a multidisciplinary team. Medical Insurance Dental Insurance Vision Insurance 401k Valid state Occupational Therapist license. NBCOT Certification preferred.
