Job Title: Physical Therapist (PT) – ContractJob Type Contract Specialty Rehabilitation Therapy Sub-Specialty: Physical Therapist Assignment Details - 13-week contract - 30 hours guaranteed per week - Shift schedule may vary - One (1) position available Practice Setting - Skilled Nursing Facility (SNF) Job Summary We are seeking a licensed Physical Therapist (PT) for a contract assignment in a Skilled Nursing Facility. The Physical Therapist will evaluate patients, develop and implement treatment plans, and help individuals restore function, improve mobility, and reduce pain through evidence-based physical therapy interventions. Responsibilities - Evaluate patients’ physical abilities and functional limitations - Develop and implement individualized physical therapy treatment plans - Provide therapeutic exercises, manual therapy, and mobility training - Monitor patient progress and modify treatment plans as needed - Educate patients and caregivers on exercises, mobility, and safety - Collaborate with an interdisciplinary healthcare team - Document evaluations, treatments, and patient outcomes accurately and timely - Ensure compliance with state regulations and facility policies Requirements - Active state Physical Therapist license (must be current) - Less than 1 year of experience accepted - Strong clinical assessment and documentation skills - Ability to work independently and in a team-oriented environment Additional Details - No bilingual requirement - No holiday coverage required - EMR system: Not specified Applications will be accepted only from candidates who meet the minimum qualifications and hold the necessary certifications.
