Job Title RN (Registered Nurse), RN, Case Manager City/State: Carmichael, California Shift: 8H Days 8hr Days, EOW, First to Float, 08:00:00-16:00:00, 8.00-5 Contract Length: 91 weeks Start Date: 03/03/2026 Job Description: Registered Nurse Case Managers coordinate patient care activities and facilitate care coordination in the acute care setting, focusing on both UR and discharge planning. Responsibilities: Provide case management services including utilization review and discharge planning. Manage initial assessments, discharge planning, and patient coordination using Interqual criteria. Collaborate with multidisciplinary teams to ensure effective communication and continuity of care. Work with all payers, including dealing with uninsured and homeless patients. Requirements: Must have prior experience with CA Dignity Health. BLS Certification required; knowledge of Cerner and Interqual preferred. BSN required with two supervisory references needed for submission.
